    IMPORTANT: If you have NOT already placed an order, please do that BEFORE you send us this information.
    IMPORTANT: We receive A LOT of orders everyday.  How do we identify you?  Please provide your Name, Paypal or Ebay or Amazon Name (not always the same as your real name), Order number, Date ordered, Items ordered...anything we can use to match your order with these instructions.
    Also please provide email address and phone number in case we need to contact you.
    Please enter this EXACTLY as you would like to see it.  Spelling is important.  ALL CAPS or Mixed Case? How many lines (if applicable)?
